Overview :

The NP reports to the Medical Director with accountability for providing strategy, judgment, organization, and evidence-based analysis to influence decisions, and directly to meet Calvado Care's Skilled Nursing requirements.

Duties & Responsibilities :

The Nurse Practitioner will have the following responsibilities :

In partnership with the clinical team, provide medical highest quality medical care for the practice’s patients.

In partnership with the Medical Director and Management, assist and support clinical personnel (e.g., clinical nurses, allied health professionals, EMR, telemedicine operations).

Provide consultations and perform diagnostic and therapeutic procedures in their clinical discipline within their practice scope, consulting when appropriate the supervising physicians for assistance with both diagnosis and treatment.

Works collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care.

Commit to the practice of value-based care which emphasizes care in the appropriate setting to improve and lower costs, address and close gaps in care and drive high patient satisfaction.

Demonstrate proficiency and support use of the practice management system and electronic medical record (EMR).

Qualifications :

Education : Nurse Practitioner Degree from an accredited U.S. school is required. Nurse practitioners must have completed accredited education and training for nursing and nurse practitioners.

An active unrestricted medical license in the State of New York is required.

Experience : At least 3 years of active practice required. DEA Certification required.

Communication : Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner.

Relationships : Ability to build and effectively manage relationships with patients, the community business leaders, and external constituents.
