We believe in supporting you both personally and professionally to help you reach your full potential. Our care model focuses on assisted living and other community-based settings, blending palliative care with internal medicine.

The Nurse Practitioners and Physician Assistants within Senior Living provide care to our highest-risk patients and are part of an interdisciplinary team.

We offer extensive training in our unique approach, and because we are not fee-for-service, our focus is on quality patient outcomes, not volume.

With a manageable expectation of approximately 4-6 patient visits per day, you’ll have the time to invest in your patients’ well-being and build meaningful relationships.

Primary Responsibilities : Conduct comprehensive assessmentsManage patients through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventionsManage patients during a short term sub-acute rehabilitation episodeEnsure accurate and complete documentation, including ICD 10 conditionsCommunicate and collaborate with the interdisciplinary care teamConduct advanced illness and advanced care planning conversationsProvide patients and caregivers with counseling and educationPerform comprehensive assessments and document findings in a concise / comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulationsFacilitate agreement and implementation of the patient’s plan of care by engaging the facility staff, families / responsible parties, primary and specialty care cliniciansCoordinate care as patients transition through different levels of care and care settingsReview orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the patient’s needs and wishesFunction independently and responsibly with minimal need for supervisionWhat Makes an Optum Career different?

Required Qualifications : For Nurse Practitioners : Active and unrestricted Nurse Practitioner license in the state which you reside, or ability to obtain by start dateGraduateof an accredited Master of Science Nursing or Doctor of Nursing Practice programBoard certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following : Family NursePractitionerAdult NursePractitionerGerontology NursePractitionerAdult-Gerontology Acute Care NursePractitionerAdult-Gerontology Primary Care NursePractitionerForPhysician Assistants : Activeand unrestricted Physician Assistant license in the state which you reside, or ability to obtain by start dateGraduate of an accredited Physician Assistant degree programBoardcertified by the National Commission on Certification of Physician Assistants (NCCPA)Active DEA licensure / prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice) (SCC04)Ability to gain a collaborative practice agreement, if applicable in your stateAbilityto complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessmentAccess to reliable transportationPreferred Qualifications : New graduates are encouraged to apply1+ years of clinical experience in practice in senior living, geriatric medicine, or home careExperience in meeting the healthcare needs of patients with complex behavioral, social and / or functional needsUnderstanding of Geriatrics and Chronic IllnessUnderstanding of Advanced Illness and end of life discussionsProficient computer skills including the ability to document medical information with written and electronic medical recordsWashington Residents Only : The salary range for this role is $88,000 to $173,200 annually.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.

Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire / rehire / transfer.

If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
