Job Summary : The Population Health Pharmacy Coordinator will support population health and quality workflows to improve medication related quality metrics by providing direct interaction / communication with patients and providers.
This role will seek to find opportunities for cost avoidance, improve quality of care, medication adherence and serve as a pharmacist extender to expand program capacity.
Minimum Education : High School diploma or GED.
Licensure, Registration and / or Certification : State Board of Oklahoma Pharmacy Technician license or have Certified Pharmacy Technician certification.
Work Experience : Minimum 3 years of related experience. Medicare Advantage and / or medical practice experience, preferred.
Knowledge, Skills and Abilities : Knowledge of medication adherence practices. Familiarity with state and federal regulations governing healthcare.
Proficient in Microsoft Excel. Good computer skills and understanding of electronic medical record processes. Excellent verbal and written communication skills.
Ability to work independently and under stringent deadlines. Understanding of CMS 5 Star program and requirements, preferred
Essential Functions and Responsibilities : Makes outbound calls to patients and healthcare professionals regarding patient medications, adherence, and barrier identification.
Conducts chart research and analysis to identify barriers to prescription regimens. Interacts with healthcare professionals of all levels involving patient medications.
Contacts the pharmacy or provider to assist patient with medication refills, medication synchronization, or 90 to 100-day supply requests.
Provides feedback and / or solutions on how to improve the medication adherence patient experience. Assists in providing patients with resources on patient assistance for medications and follows up as necessary.
Triage patients to pharmacist for medication education / to resolve medication issues or to primary care provider’s office for other items.
Decision Making : Independent judgment in making minor decisions where alternatives are limited and standard policies / protocols have been established.
Working Relationships : Works directly with patients and / or customers. Works with internal / external customers via telephone or face to face interaction.
Works with other healthcare professionals and staff.
