Overview
HarmonyCares is seeking a dedicated Hospice RN Case Manager to provide high-quality, coordinated health care in the home.

Responsibilities
- Admit patients eligible for hospice services under physician's order
- Assess patient needs and formulate individualized care plans
- Communicate patient progress with other disciplines and supervises hospice aide staff
- Update care plans and address patient problems as changes occur
- Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications
- Current unencumbered RN license in the state of practice
- Valid driver's license and good driving record
- Ability to make sound nursing judgments and formulate care plans
