Care Manager RN - Cardiology Institute

Description

The Registered Nurse Specialist is responsible for managing patient populations with chronic illness and / or multiple co-morbidities.

This position works in coordination with multiple PCPs and serves as the central link to an identified population of patients.

Required Qualifications :

• Graduation from an approved, accredited school of Nursing
• Upon hire : Washington Registered Nurse License
• 2 years Work experience in an RN role
• Work experience revolving around medications and IV solutions in terms of purpose, administration, side effects and contraindications and administers according to established standards and provider orders
• Experience managing complex patients in an ambulatory setting

Preferred Qualifications :

• Data analytics and previous cardiology experience preferred
• Two years of experience a combination of hospital & ambulatory services preferred
