Licensed Practical Nurse #Full Time

Job Summary :

The Licensed Practical Nurse (LPN) will serve as a key member of the patient care team in a Primary Care ambulatory setting.

The LPN will work closely with Nurse Practitioners across as well as with other clinical and administrative staff to deliver an exceptional patient experience that contributes to positive health outcomes for patients and a work culture of kindness, inclusion, integrity, accountability, and excellence.

The LPN is responsible for assisting the nurse practitioners by providing general nursing care, patient screening, and clinical support.

The LPN will also provide patient education and medication administration (including injections) as directed by the nurse practitioner(s).

Job Responsibilities :

• Tracks abnormal lab results to confirm that patient has had appropriate follow up as ordered by the provider in the patients plan of care
• Perform point of care testing
• Triage clinically-related calls and assist with EPIC clinical in-basket messages / telephone encounters and advise appropriate clinicians and provide basic nursing recommendations and making appointments
• Assist in scheduling patients for follow-ups and sick visits
• Maintains precise records of all specimens obtained and sent for testing
• Enter / review orders and document clinical notes in EPIC as advised by NP providers
• Notifies patients of normal results as determined by the provider either telephone or the Connect patient portal
• Obtain pre-certifications / pre-authorizations for medications, services and testing in a timely manner when necessary
• Administer immunizations as ordered by providers
• Serve as chaperone during sensitive exams
• Provides NPs with results for routine review prior to scanning
• Provides clinical assistance to clinical coordinator with phlebotomy
• Provides backup coverage for front desk and medical assistants, when required
• Assist with care coordination of patient needs
• Contact durable medical equipment companies and place orders as directed by NPs
• Miscellaneous duties as assigned

Job Qualifications :

• High school graduate or equivalency and graduated from an accredited Practical Nursing program required.
• 2 years' relevant experience.
• Current NYS LPN license in good standing required.
• Current BCLS / AED certification required.
• Must demonstrate proficiency of OSHA standards including disposal of biohazard waste.
• Strong proficiency in computer skills including, but not limited to, Electronic Health Systems, Microsoft Excel, Microsoft Word, Email systems, etc.
• Phlebotomy certification or training a plus.
• Preferred primary care experience in a high volume setting preferred
• Must demonstrate strong organizational and problem-solving skills.
• Must have excellent communication skills, interpersonal skills, and a strong ability to de-escalate stressful situations.
• Phlebotomy, EKG, and GYN experience preferred
• Bilingual Spanish preferred
• Prior EMR experience, EPIC preferred.

Hourly Rate Ranges : $28.37 - $44.23

Note : Our salary offers will fall within these ranges based on a variety of factors, including but not limited to experience, skill set, training, and education.
