About The Position
Job Title : Registered Nurse Job Specialty : Rehabilitation - General Rehabilitation Job Duration : 13 weeks Shift : 8hr Day Shift (40 hours per week) Guaranteed Hours : Not specified Experience : Minimum 1 year of home health RN experience License : IL RN license Certifications : BLS (AHA) Must-Have : Driver's license, insurance, and reliable transportation- Ability to work independently- Solid clinical and critical thinking skills Job Description : Perform home visits with 5-6 patients a day (general Med / Surg type adult patients).
- Provide care to patients in Lake County, specifically northeastern regions.- Maintain flexibility in dress code; any scrub color or business casual accepted, excluding jeans or open-toed shoes.
- Ensure all travel and patient visits are completed within scheduled shifts.- Deliver high-quality, patient-centered care with a focus on rehabilitation.
- Utilize effective communication skills to interact with patients, families, and healthcare team members.- Demonstrate strong organizational skills and the ability to manage multiple priorities.
- Maintain accurate and timely documentation of patient care and activities.
