We’re thrilled to offer a $5,000 sign-on bonus and a relocation package of $5,000 (for moves over 100 miles) or $2,500 (for moves over 50 miles) for experienced nurses joining our team. This opportunity is exclusively for external applicants with 1 or more years of nursing experience.

Position Overview :

This role will primarily cover the Carle Health South Clinic and is a hybrid position allowing for 1 day of remote work and 4 days in the office. As an Outpatient Care Management RN, you will play a vital role in providing care management and population health services to our patients, particularly those with chronic diseases or high vulnerability during transitions between care settings.

Your Responsibilities Will Include :

• Coordinating care for patients with chronic diseases and facilitating medication management.
• Creating personalized, longitudinal care plans tailored to each patient’s unique needs.
• Building effective partnerships with patients, families, and healthcare professionals.
• Utilizing a data-driven approach to enhance patient access to care and improve clinical outcomes.
• Conducting holistic health assessments to understand patient needs and goals.
• Documenting clinical and financial information accurately in the Electronic Medical Record (EMR).

Compensation and Benefits :

The compensation range for this position is $30.39 to $52.27 per hour . This range reflects a good faith estimate at the time of posting and may vary based on experience, qualifications, and other factors.
