Posting Summary Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for Mercer North for the Child Health Program within the School of Nursing, located in Lawrenceville, NJ.
The Heath Care Case Manager (HCCM) functions to support the Department of Children and Families (DCF) / Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office.
Among the key duties of this position are the following :
- Upon entry into out-of-home placement, assists with enrollment of a child in a managed care entity and identification of a medical home (primary care provider), and informs the child, caregiver and DCF / CP&P, as needed.
- Assesses a child's heath care status and needs by retrieving a complete health history through obtaining and reviewing all medical records and interview
