Georgia Licensed Care Manager (RN)

A company is looking for a Care Manager (RN) who will develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

Evaluate member needs and recommend care plans to achieve optimal health outcomes

Collaborate with providers and community resources to address members' unmet needs

Monitor and update care plans based on member progress and changing conditions

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience

RN - Registered Nurse - State Licensure and / or Compact State Licensure required

Certified Diabetes Educator - Preferred
