Position Details :
We are seeking an experienced Advanced Practitioner for our REACH Program serving geriatric patients in the greater Fairfield area.
Role : Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA)
Location : Homecare based in the greater Fairfield, CT area
Setting : Geriatric Medicine in the home setting, working within a multi-disciplinary care team
Schedule : Full-time, weekdays

Qualifications
Requirements :
- Licensure : Unrestricted State of Connecticut Medical License (or eligible)
- Training : APRN (Family, Adult / Geri Nurse Practitioner) or PA
- Experience : At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)
