Join our team as a Hospice RN Case Manager and make a difference when life matters most!
As a member of our compassionate team, you will be responsible for providing direct patient care services to hospice patients and supportive care to the patient's family.
You will act as the primary liaison between physician, patient / family, and the hospice team, as well as manage and collaborate with an interdisciplinary group to provide comfort, peace, and dignity to our patients and their families.
You will also participate in company quality assessment and performance improvement programs.
Requirements :
- Hospice heart : compassionate, kind, and empathetic to patients and their families.
- Comfortable providing care wherever the patient calls home.
- RN license in the state of requested employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Current driver's license and automobile insurance.
- Current CPR certification.
