RESPONSIBILITIES :
Case Management Nurses (Case Mgmt RN) coordinate long-term individual care services beyond the bedside, providing long-term assistance to patients and family members to advocate for their quality care.
Case Mgmt RN job responsibilities include, but are not limited to :
Pair patients with resources they need within or outside current facilities, finding affordable, efficient healthcare services that suit every patient’s unique medical condition
Document case management plans while setting goals for each patients’ treatment
Case Mgmt RN Collaborate with a variety of healthcare providers offering the resources to help their patients
