Summary
As a Home Care RN Case Manager, you'll be responsible for providing comprehensive nursing care to assigned patients, assessing their needs, developing and implementing care plans, and coordinating interdisciplinary services.
This position requires working independently and prior nursing or home health experience is preferred.
Location : Ontario / Seneca County
Hours : Full Time, Monday-Friday Day Shift with travel to Yates and Cayuga if needed
Minimum Qualifications
- Associate's degree in Nursing, 1 year of nursing experience preferred
- Prior home health or clinical experience preferred
- Ability to work independently
Responsibilities
- Patient Care & Service : Develop and implement interdisciplinary care plans, manage and coordinate patient care, communicate changes in patient status, and maintain accurate documentation
- Compliance : Ensure privacy of patient information in accordance with corporate policies
Physical Requirements : Medium work requiring frequent walking, standing, or squatting
Pay Range : $72,800.00 - $97,000.00
The listed base pay range is subject to modification and additional pay components based on experience, qualifications, and other factors.
