At Houston Methodist the Pharmacist Residentposition is responsible for participating in an organized directedpostgraduate training program that centers on development of theknowledge attitudes and skills needed to pursue rational drugtherapy for a generalized populations as well as specializeddiseases populations.

This position performs fundamental clinicalpharmacy duties and facilitates safe efficacious andpharmacoeconomically rational use of medications in thepopulation(s) served.

The Pharmacist Resident position supports thedelivery of pharmacy managed protocols formulary processes andmedication reconciliation activities while providing education tofellow pharmacists nursing medical staffs and others inside andoutside the department.

This position has additional pharmacyadministrative quality and research activities that are part of thetraining program requirements.

A certificate of residency isgranted to the Pharmacist Resident upon satisfactory completion ofthe residencyprogram.

PEOPLEESSENTIAL FUNCTIONS

Promotesa positive work environment and contributes to a dynamic teamfocused work unit that actively helps one another to achieveoptimal department results.

Collaborates with other health careprofessionals as needed for problem resolution and / or achievementof identified patient specificgoals.

Serves as preceptorand mentor for pharmacy students. Completes evaluation forms andassigns rotationschedules.

SERVICEESSENTIAL FUNCTIONS

  • Providesfundamental pharmacotherapy management to patients. Activitiesinclude but are not limited to proactive drug therapy reviews andmonitoring providing drug information patient counseling andmanagement of pharmacistbased dosingprotocols.
  • Exercises goodjudgment in patient care decisions per performance standard anddocuments patient care activities and patient outcomes as impactedby pharmaceutical care and clinicalinterventions.
  • Serves as aguest or member on one or more Medical Staff committees andactively contributes to accomplishing the goals of the committee orsubcommittee.

QUALITY / SAFETYESSENTIALFUNCTIONS

Collaborates withother health care professionals to manage patients drug therapy foreffectiveness side effects adverse drug reactions toxicities druginteractions and incompatibilities following guidelines forformulary management and clinical intervention tools in theachievement of patient specific goals.

Documents patient careactivities and patient outcomes as impacted by pharmaceutical careand clinicalinterventions.

  • Completesrotation objectives and all assigned duties described in theidentified specific residency program by demonstrating competenciesthat may include but are not limited to drug information and policydevelopment acute patient care ambulatory patient care researchproject oversight practice management and weekendcoverage.
  • Participates indepartmental quality management activities i.e. medication useevaluations adverse drug reactions and medication error reportingclinical interventions medication center reviews and inventorymanagement documentation andreporting.

FINANCEESSENTIALFUNCTIONS

  • Contributes to theprovision of pharmacoeconomically sound medication therapy byupholding HM Formulary standards in area(s) ofresponsibility.
  • Selfmotivatedto independently manage time effectively and prioritize tasks forself and for specialty area offocus.

GROWTH / INNOVATIONESSENTIAL FUNCTIONS

Conducts a majorresearch project to advance the pharmacy departments goals and thepharmacy profession. Submits research and scholarly work forpublication platform presentations and / or poster presentations.

Gives presentations at local state and national meetings includinginservices presentations to pharmacy medical nursing and nutritionstaff in area(s) ofresponsibility.

Participatesin inter and intradepartment activities including inservicesarticle publication / publication preceptorship and additionaltraining.

EDUCATION

  • Graduatefrom an accredited College of Pharmacy or recognition as a ForeignPharmacist Equivalent Graduate by the Texas State Board of Pharmacy
  • Transcriptsmust be provided during the application process whenrequested

WORKEXPERIENCE

Zero experience;some experience in a hospital setting highlypreferred

LICENSESAND CERTIFICATIONSREQUIRED

  • PHRM PharmacistState Licensure Texas Pharmacist license or eligible for TexasPharmacist license within 90 days AND
  • BLS Basic Life Support(AHA) within 90 days AND
  • ACLS Advanced CardiacLife Support (AHA) within 90 days AND
  • PALS Pediatric AdvancedLife Support (AHA) for pharmacists regularly assigned to respond toCode Blue events within 90 days AND
  • NRP NeonatalResuscitation if applicable to primary work assignment within 90days AND
  • IV CertificationIntravenous certification (ACPE) if applicable to primary workassignment within 6 months

  • Demonstratesthe skills and competencies necessary to safely perform theassigned job determined through ongoing skills competencyassessments and performanceevaluations

  • Demonstrates theknowledge of basic principles regarding rational drug therapyincluding but not limited to professional and technical competencein the selection evaluation utilization and distribution of drugsand drug information and apply these to all patient groups neonateto geriatric
  • Experiencewith comprehensive pharmacy services including unitdose drugdistribution intravenous admixture preparation nutrition supportservices and target drugmonitoring
  • Competence inresearch in an area related to the practice of pharmacy andmaintains active credentialing with HM Academic Institute asapplicable to primary work assignment (refer to manager fordetails)
