About The Position
Job Title : Rehabilitation Therapist Job Specialty : Physical Therapist Job Duration : 13 weeks Shift : Monday-Friday, Days Guaranteed Hours : 32 hours per week Experience : Open to new graduates, SNF experience preferred License : State License required Certifications : None specified Must-Have : Completion of the prescreen form, resume, and license verification before submission Job Description : - Complete and attach the prescreen form when submitting a candidate.
- Attach resume and license verification for consideration. - Maintain compliance with the following credentialing checklist prior to submission : - State License Verification - Resume - Three professional references - County Criminal Check (7 years for all counties lived and worked in) - Federal Criminal Check (7 years or more) - Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS / OIG Search - GSA Search - Social Security Number Trace - 1 Step PPD or Quantiferon, or proof of positive PPD and chest X-ray - SAM Search - Virginia State Police Background Check - Mandated Reporter Form - Sworn Statement - A Covid test is required 72 hours prior to the start date with results issued 48 hours before.
- Position urgency is considered normal. - No holiday coverage required. - Bilingual ability is not necessary.
