CRITERIA BASED JOB DESCRIPTION

POSITION SUMMARY :
This position is based on the treatment philosophy of working with families in a strength based wrap-around model, and the belief that children are usually best served in their homes and community using natural supports and defining their own vision.
A Care Manager coordinates the assessment, planning and treatment of services for moderately / severely emotionally disturbed children and their families.
1. Receive and review referral information and all existing assessments prior to the coordination of the initial team meeting and then initiate the case file.
2. Meet with the child and family within an established timeframe to gather additional information, orient the family to the Individualized Service Plan process and wraparound philosophy, establish a positive relationship, and identify additional team members.
3. Coordinate the scheduling of all team meetings and participate in the formulation of a culturally sensitive Individualized Service Plan according to established standards and within established timeframes.
4. Complete the Individualized Service Plan within established timeframe and update as per standards in order to support timely submission to the Contracted Systems Administration for authorization of services.
5. Link the family to designated services as identified in the Individualized Service Plan.
6. Support Individualized Service Plan goals and objectives.
7. Ensure the implementation of the crisis management plan and make timely necessary revisions.
8. Document all contact, intervention, and status updates in the case record according to established standards.
9. Provide scheduled on-call crisis management coverage.
10. Maintain a log of all contacts and prepare a monthly report for submission to Care Manager Supervisor.
11. Monitor wraparound service providers to verify provision of authorized services to receive feedback regarding progress toward goals, and to support positive collaboration.
12. Meet with the Care Manager Supervisor for supervision.
13. Identify gaps in formal services and provide this information to Supervisor.
14. Attend educational trainings and conferences. Keep skills up to date.
15. Demonstrate professionalism and sensitivity when interacting with child / family and co-workers.
16. Uphold appropriate levels of confidentiality.
17. Maintain respect for youth and family's rights.

POSITION QUALIFICATIONS
MINIMUM EDUCATION : BSW / CSW or BA in a related mental health field required.
MINIMUM EXPERIENCE : A. Prior experience working with emotional behavioral challenges, developmental disabilities, and substance use, preferably within a large children’s serving system.
B. Bilingual (English / Spanish or English / Arabic) a plus.
C. New Jersey driver’s license in satisfactory standing.
SKILLS : Computer literate
