Summary
We are seeking a highly motivated and experienced Diabetes Nurse Practitioner to provide specialized, comprehensive care and education to patients with diabetes. The NP will function as a vital member of our care team, focusing on improving patient outcomes and empowering patients in the self-management of their chronic illness.
Key Responsibilities
- Conduct thorough patient assessments , including health history and physical examinations, for individuals with all types of diabetes.
- Diagnose and manage acute and chronic issues related to diabetes.
- Develop, implement, and adjust individualized treatment plans , including the prescription and management of medications, such as insulin and oral anti-diabetic agents.
- Order, perform, and interpret diagnostic tests (e.g., lab results, glucose monitoring data, insulin pumps).
- Provide extensive patient and family education on disease self-management, including nutrition, physical activity, medication adherence, and complication prevention.
- Instruct patients on the use of diabetes management technologies, such as continuous glucose monitors (CGM) and insulin pumps .
- Maintain accurate, timely, and confidential medical records in the Electronic Health Record (EHR).
- Participate in quality improvement initiatives and stay current with evidence-based practice standards in diabetes care.
Qualifications
- Education : Masters degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program.
- License / Certification :
Current, unrestricted Advanced Practice Registered Nurse (APRN) license in the state of Maryland.
- Active Prescriptive Authority (DEA registration, if applicable).
- Basic Life Support (BLS) certification required.
- Preferred : Certification as a Certified Diabetes Care and Education Specialist (CDCES) or Board Certified in Advanced Diabetes Management (BC-ADM).
- Experience : Minimum of 1-2 years of experience as an NP, preferably in endocrinology, diabetes, or chronic disease management.
- Skills : Excellent communication, counseling, and collaboration skills; ability to work independently; proficiency with EHR systems.
