Summary Resilient Healthcare combines virtual and in-person care, utilizing specialized technology to better enhance and streamline the experience for both the patient and the clinician.
- Reporting to the Medical Director, the Advanced Practice Provider will plan, direct, and evaluate health care need in the home setting and in a hospital at home model;
- examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries / illnesses; performs selected laboratory testing;
offer referrals to physicians, write prescriptions, and perform home procedures within the scope of expertise, protocols, and available equipment.
Essential Job Functions The ideal candidate will have excellent interpersonal skills to assist with physician and interdisciplinary team member communications.
This person is responsible for providing in-home care to patients across the spectrum of acuity. The candidate will also embrace a new healthcare delivery system that will work to improve the overall availability of healthcare services.

1. Meets with new patients in their home to orient them about services, to identify needs, and to suggest physicians and services.
2. Sees hospital at home patients for laboratory testing, examination, diagnosis, and treatment of chronic and acute episodic illnesses and minor injuries.
Refers to physician as needed. Writes prescriptions and performs office procedures within the scope of expertise, protocols, and available equipment.
Makes house calls and telehealth visits as needed. 3. Performs physical examinations. 4. Conducts support groups in subjects related to physical and emotional health (i.
e., challenge of change, dealing with loss, dealing with chronic illness as caregiver, etc.). Coordinates health promotional activities (providing printed information, routine testing, flu shots, mammograms, etc.
5. Acts as a resource person for decisions, planning, and policies affecting the health of patients. 6. Maintains patient records.
Records statistics and conducts research as deemed necessary by the Company. Compiles reports concerning care at patient’s home.
7. Collaborates with staff to identify patients' needs and to help develop plan to meet these needs. 8. Follows patients in hospital at home as needed for continuity of care.
9. Serves as a member of committees dealing with health care. 10. Able to navigate technology for telehealth visit. 11. Other duties as assigned.
Minimum Qualifications 1. Master’s Degree in Science of Nursing or Physician Assistant Studies or related required 2. Minimum 3 years of advanced practice provider experience required 3.
Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas required 4. Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants required 5.
American Red Cross or American Heart Association Basic Life Support Course (BLS or BLCLS) and Certification 6. American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification 7.

An individual in this position may be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (i.
e. needle sticks, etc.) and chemical hazards. Travel by automobile required. Equipment Used This role routinely uses standard office equipment such as computer, keyboard, telephone, fax machine, copy machine, calculator.
Additionally, this role routinely uses medical equipment such as blood pressure cuff, thermometer, stethoscope, etc. Physical Requirements While performing the duties of this job, the employee is regularly required to stand, walk, reach with hands and arms, twist, stoop, and kneel.
