Essential Job Functions / Responsibilities :
- Conduct assessments, treatment planning, evaluation of the response to care and readjustment of the treatment plan as needed;
- Plan and provide community education to patients, families and peers;
- Accurately document patient care treatments in EHR (i.e., HomeCare HomeBase);
- Determine a treatment plan
Position Qualifications :
- 1 year physical therapy experience;
- Current & valid CA PT license; and
- American Heart Association Basic Life Support (BLS) certification.
Travel Physical Therapist ($2800-$3499 per week)
