About the Organization
Community Physical was founded in 1986 by Bob Tripicchio, PT, MS, DSc, on the premise that people are the most important asset of any organization.
CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services.
Throughout this growth period, CPT has remained focused on maintaining a practice that creates values for employees so as to leverage the best possible knowledge possible.

PRN ST need
At CPT, we have developed a Continuum of Care (COC) program that offers home care and outpatient services for Alden Assisted Living and Independent Living Communities residents.
Our home care services are provided through Family Home Health and are documented on Kinnser, while our outpatient services are documented on ThereaOffice.

King Bruwaert House, located in Burr Ridge, is an independent living / assistant living community that includes a 48-bed 'SNF' non-Medicare bed building, a 78-resident AL building, an 11-resident Memory Care site, and 2 IL campuses.
The 'SNF' is required to have care plans and must meet Medicare requirements, but we do not accept Med A admits.
Do you have experience in homecare and outpatient services? Are you familiar with TheraOffice and Kinnser? Are you available to see patients during normal business hours, Monday through Friday, from 7 a.m. to 6 p.m.?
