About The Position
Job Title : Registered Nurse Job Specialty : Case Manager Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 16 : 30) Guaranteed Hours : Not specified Experience : Minimum 1 year of recent experience with discharge planning as a Clinical Case Manager with a focus on acute care hospital settings License : Registered Nurse License Certifications : Basic Life Support (BLS) Must-Have : - Acute hospital experience in case management and discharge planning - Prior travel experience - Willingness to work in weekend rotation - Experience with InterQual, EPIC, Allscripts / Careport Job Description : - Perform clinical case management with an emphasis on discharge planning in an acute care hospital setting- Utilize skills and knowledge in equal facility coordination and patient transition planning - Engage in collaborative practice with healthcare team members - Ensure effective communication with patients and family members for safe discharge- Experience in inpatient case manager roles to manage care continuum - Completion of compliance documents and mandatory modules prior to start - Participation in a structured orientation for seamless transition into role Candidates must fulfill compliance requirements promptly, and modules to complete during orientation are necessary for onboarding.
