Purpose :

The primary responsibility of this role is to submit care gap information to payer partners and develop projects to ensure patients receive appropriate preventive screenings and manage their conditions effectively.

Additionally, this position offers exposure to a variety of opportunities within the healthcare field, allowing you to gain experience in healthcare governance, finance, management, and project management.

This role is ideal for individuals looking to contribute to quality improvement while exploring broader aspects of healthcare administration.

Essential Duties / Responsibilities :

  • Close Clinical Care Gaps : Utilize designated processes to identify and address gaps in patient care, ensuring timely interventions.
  • Collaborative Meetings : Participate in regular team meetings to discuss objectives, review progress, and reflect on outcomes.
  • Project Support : Prepare, organize, and maintain documentation for project deliverables and action items, ensuring accuracy and completeness.
  • Quality Reporting : Assist in the creation, publication, and distribution of quality reports to drive continuous improvement.
  • Patient Engagement : Contribute creative outreach ideas to engage patients, enhancing their involvement in improving the quality of care.

Other Duties :

  • Support ACO Initiatives : Aid with various Accountable Care Organization (ACO) initiatives as needed.
  • Confidentiality : Uphold strict confidentiality standards in all aspects of work.
  • Professionalism : Always maintain a professional appearance and demeanor.
  • Punctuality : Arrive on time and be prepared for work and meetings.
  • Team Collaboration : Actively engage as a team member, supporting decisions, embracing change, managing conflict effectively, and valuing the contributions of others.

QUALIFICATIONS

  • Fast-Paced Environment : Ability to thrive in a fast-paced setting, managing multiple tasks efficiently.
  • Technical Proficiency : Experience in Microsoft Word, Excel, and Outlook is required.
  • Teamwork : Strong ability to work collaboratively within a team environment.
  • Organizational Skills : Excellent organizational and time management skills, with keen attention to detail.
  • Communication : Strong oral and written communication skills, with the ability to articulate ideas clearly and effectively.

Required :

  • Educational / Experience Combination : A combination of relevant education and / or administrative experience.
  • Availability : Ability to work 15 to 20 hours per week during normal business hours.
  • Attention to Detail : Demonstrated ability to maintain high accuracy and timeliness in completing tasks.
  • Compassion and Care : Ability to show empathy and care when interacting with patients and colleagues.
  • Problem-Solving : Strong problem-solving skills with the ability to identify issues and implement effective solutions.
  • Listening Skills : Ability to listen attentively to patient needs and respond appropriately.
  • Conflict Resolution : Proficiency in mediation and conflict resolution techniques.

Preferred (Not Required) :

  • Technical Skills : Familiarity with computer systems and applications, particularly Microsoft Office and Excel.
  • Patient Care Experience : Previous experience in outreach, patient care coordination, and customer service within a medical setting.
  • Healthcare Knowledge : Understanding of medical terminology, healthcare field dynamics, and medical specialties.
  • Motivational Interviewing : Knowledge of motivational interviewing techniques is an asset.
  • Appointment Scheduling : Experience in scheduling patient appointments within a medical office or clinic environment.
