Purpose :
The primary responsibility of this role is to submit care gap information to payer partners and develop projects to ensure patients receive appropriate preventive screenings and manage their conditions effectively.
Additionally, this position offers exposure to a variety of opportunities within the healthcare field, allowing you to gain experience in healthcare governance, finance, management, and project management.

Essential Duties / Responsibilities :
- Close Clinical Care Gaps : Utilize designated processes to identify and address gaps in patient care, ensuring timely interventions.
- Collaborative Meetings : Participate in regular team meetings to discuss objectives, review progress, and reflect on outcomes.
- Project Support : Prepare, organize, and maintain documentation for project deliverables and action items, ensuring accuracy and completeness.
- Quality Reporting : Assist in the creation, publication, and distribution of quality reports to drive continuous improvement.
- Patient Engagement : Contribute creative outreach ideas to engage patients, enhancing their involvement in improving the quality of care.
Other Duties :
- Support ACO Initiatives : Aid with various Accountable Care Organization (ACO) initiatives as needed.
