Territory is Sandy Springs, North Fulton County, Midtown, and West Atlanta.
Home Health RN Job Details
- Pay Rate : $2411-$2611 weekly gross
- Schedule : 8a - 5p
- Available Shift : 8 hour shifts, 5 shifts per week
- Start Date : 9 / 16 / 24
- Located In : Atlanta, GA
- Requirements : Qualified applicants MUST have at least 1 years of experience in a Medicare-certified home health agency, have a valid RN license and be willing to obtain GA licensure.
Responsibilities
As a Registered Nurse (RN), you'll have the opportunity to provide personalized nursing services to patients in the comfort of their homes, fostering independence and improving their quality of life.
This one-on-one healthcare approach allows you to connect and collaborate with individuals on their care, while acting as an advocate for their needs and quality of life. Responsibilities include :
- Conducting thorough assessments of patients’ clinical needs and developing individualized treatment plans to address their unique needs
- Demonstrate knowledge and competence in advanced practice nursing skills, including the nursing process, and collaborating with interdisciplinary team
- Demonstrate knowledge of home health proficiency in quality of clinical records, productivity, and patient satisfaction
- Documenting patient evaluations, progress notes, and treatment outcomes accurately and promptly in electronic medical records
- Monitoring patients’ progress and adjust treatment plans as needed to achieve care goals and optimize outcomes
In the Home Health setting, you will be a vital link between a patient and their family to resources across the care spectrum.
