Synapticure is the largest, most comprehensive specialty care clinic in the country for those living with neurodegenerative diseases.

Founded by patients and caregivers living with these diseases, we are redefining the care journey for diseases like Alzheimer’s, Parkinson’s, and ALS in all 50 states.

With a virtual care model, we democratize flexible, specialized care for patients and caregivers across the country. Our team creates personalized guided journeys focused on education and empowerment, while relentlessly working to facilitate access to clinical trials and expert multidisciplinary care providers like neurologists, psychologists, and care coordinators who collaborate to provide comprehensive patient care.

To serve the millions of patients and caregivers living with neurodegenerative diseases, Synapticure requires determined, compassionate team members who are eager to engage patients who would benefit from Synapticure’s care.

You thrive on direct communication with patients, caregivers and their loved ones and have a passion for working with underserved populations.

You are thoughtful, organized, curious, empathetic, and kind. You welcome the opportunity to impact someone’s life positively.

Your Role

As an Outreach Specialist, you are responsible for locating and connecting with patients who would benefit from Synapticure’s care model, and engaging with them around their needs.

However, until the transition to their Care Coordinator is made, you are the patient’s point person, available and eager to assist with whatever they need : support, education, or a listening ear.

You work alongside, and are supported by, Synapticure’s care team to ensure patients and caregivers have the support they deserve, right from the start.

  • Working daily within EMR to collect data, organize information, track tasks, and communicate with staff, patients, and family members;

Providing feedback to the product development team, and, over time, becoming part of a super-user group to assist in onboarding and supporting others
