We’re dedicated to making every day brighter for our memory care residents, and we’re looking for an enthusiastic Licensed Practical Nurse (LPN) to join our dynamic team.

If you're passionate about delivering top-notch care and ready for an exciting opportunity, we’d love to hear from you! What You'll Do : Distribute medications with precision and care, ensuring each resident receives the treatment they need.

Provide hands-on care and support to our memory care residents, enhancing their quality of life. Help assess and evaluate patients’ diverse needs, collaborating with our multidisciplinary team to achieve optimal outcomes.

Contribute to the development and implementation of care plans and discharge plans tailored to individual needs. Facilitate and provide educational opportunities for patients and their families based on customized teaching plans.

Adapt to the ever-evolving healthcare environment, embracing new technologies and processes with a positive attitude. Relay important patient changes to the registered nurse and multidisciplinary team to ensure seamless care.

Manage and utilize department resources efficiently, supporting staffing needs and contributing to process improvement. Accurately document patient information in both electronic and paper medical records in a timely manner.

Uphold HIPAA regulations and maintain confidentiality at all times. What We’re Looking For : Qualifications : Licensed Practical Nurse (LPN) with Basic Life Support (BLS) certification.

Experience : Prior experience in memory care or similar settings preferred. Skills : Strong clinical skills, effective communication, and a proactive approach to patient care.
