We have an exciting opportunity to join our team as a Quality Improvement Manager , RN , CPHQ - Hemodialysis-Main St - - Full Time days - Monday - Friday.

In this role, the successful candidate Responsible for the initiation and completion of performance improvement projects as identified by Network Managers, the ESRD Network Quality Improvement Committee and CMS throughout the departments.

Additional focus will be liaison to Hospital-wide Nursing Quality Improvement and Quality Management and assist in activities required by the ESRD Network.

Job Responsibilities :

Responsible for Quality Assessment Performance Improvement (QAPI). Provides data for trending of causes of mortality and hospitalizations.

Inputs data for the Centers for Disease Control (CDC) National Healthcare Surveillance Network.

Completes special projects as identified by QAPI committee, Administrative Director or Performance Improvement Specialist.

Participates in inspection preparation throughout the network. Disseminates QAPI meeting minutes and information to appropriate personnel throughout the Nephrology network.

Provides assistance as needed to QAPI subcommittee process owners.

  • Provides Patient Care, as needed. Able to provide direct patient care within scope of practice throughout the
  • network as required.
  • Participates in collaborative efforts to ensure that patient safety and error reduction strategies are met.
  • Ensure staff adherence to department and hospital policies.
  • Ensures that quarterly chart audits for all locations and patients are reviewed for accuracy and compliance. Ensures charts are complete with required forms and respective backup information attached.
  • Assumes manager on-call rotation as required.
  • Facilitates unit ESRD Network counsels.
  • Collaborates with staff in education and implementation of computer upgrades. Collaborates with computer vendor in maintenance and upgrades to program.
  • Motivates and fosters collaborative team efforts. Educates, advises and guides clinical and support departments in developing effective Quality Improvement Plans and Quality Assessment Performance Improvement Teams.
  • Facilitates process improvement initiatives within the department to enhance the quality and safety of patient care.
  • Maintains organized files. Makes recommendations for improvement. Coordinates generation and communication of reports to appropriate departments and staff members.

Collaborates with the Centers for Medicare & Medicaid Services and IPRO ESRD Network of New York in coordination of data collection and distribution of reports.

Minimum Qualifications :

To qualify you must have a Bachelor's Degree In related field. Current NYS Registered Nurse License. Professional Certifications / Registration :

Current NYS Certified Professional Healthcare Quality Certification (CPHQ) preferred. 1 - 2 years related field. Excellent communication (both oral and written), interpersonal, organization, leadership skills.

Demonstrated competency in the care of patients with renal disease, interpersonal relationships and management..

Required Licenses : Registered Nurse License-NYS

Qualified candidates must be able to effectively communicate with all levels of the organization.

Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements.

The salary range for the role is $79,310.00 - $134,157.50 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.

The salary range or contractual rate listed does not include bonuses / incentive, differential pay or other forms of compensation or benefits.
