Job Summary :

We are seeking a dedicated and experienced Registered Nurse (RN) – Home Health Case Manager to join our growing team. In this role, you will coordinate and deliver skilled nursing care to patients in their homes, managing a caseload and ensuring high-quality outcomes through individualized care plans. The ideal candidate will have strong clinical judgment, excellent communication skills, and a compassionate approach to patient care.

Key Responsibilities :

Provide skilled nursing care and case management to home health patients

Perform initial and ongoing assessments of patient needs

Develop, implement, and update individualized care plans in collaboration with physicians and the interdisciplinary team

Monitor patient progress and adjust care plans accordingly

Coordinate services and referrals to other disciplines (PT, OT, MSW, etc.) as needed

Educate patients and families on diagnoses, treatment plans, and medications

Maintain accurate and timely documentation in compliance with OASIS, Medicare, and agency standards

Participate in case conferences and contribute to quality improvement initiatives

Ensure continuity of care by communicating effectively with healthcare providers, patients, and caregivers

Comply with all agency policies, procedures, and infection control guidelines

Qualifications :

Current RN license in the state of [Insert State] (in good standing)

Minimum 1–2 years of RN experience; home health experience required or strongly preferred

Strong knowledge of OASIS documentation and Medicare guidelines

Current BLS certification

Valid driver’s license and reliable transportation

Excellent assessment, organizational, and time management skills

Ability to work independently and manage a caseload efficiently

Preferred :

Experience with electronic medical records (EMR) such as Kinnser, Homecare Homebase (HCHB), or similar systems

Certification in home health nursing (CHPN, CHRN) is a plus
