About The Position

- Skilled Nursing Facility (SNF) experience preferredLicense : State License Verification requiredCertifications : COVID-19 test required 72 hours prior to start date with results issued 48 hours priorMust-Have : Prescreen filled out and attached, resume, and license verificationJob Description : - Ensure the prescreen is filled out and attached when submitting the application- Attach a resume and license verification;
incomplete submissions will result in the candidate being declined- Conduct a state license verification and provide three references- Complete background checks including County Criminal (7 years for all counties lived and worked), Federal Criminal (7 years or more), Multi-State Criminal Search (7 years or more), and Nationwide Sex Offender Search (7 years or more)- Perform HHS / OIG, GSA, and SAM searches- Provide a Social Security Number Trace - Submit proof of 2-step PPD or Quantiferon, or proof of positive PPD and chest X-ray - Undergo Oregon State Background - CRIMS (facility run)Note : Position urgency is normal, and bilingual or holiday coverage is not required.
