In this role you will serve on aninterdisciplinary team (IDT) and provide direct medical care toPACE participants. You will work collaboratively with the IDT tocreate and drive forward a care plan that improves health outcomesfor your panel of participants.
In this role you will meetparticipants where they are whether that be in the center clinichome virtually or an external site of care.
KeyResponsibilities :
Provide direct medical care toPACE participants
Complete sections ofassessments and care planning
Manage a panel ofparticipants
Initiate evaluate review andchange orders for medications and diagnostic studies or treatmentswith approval
Assess initiate and coordinatecare on a daily basis under supervision
Document all participant interactions assessments next steps in theEMR within the specified timeframe set by the center
Make referrals to specialists with approval andrequest consults as needed
Perform history andphysicals and participant teaching
Orderinterpret and follow up on lab tests and results
Perform selected procedures Recognize andescalate abnormalities as needed
Liaise withother team members for participants care including behavioralhealth and pharmacy
Evaluate and treatparticipants where they are located whether that be in the centerhome virtually SNF etc
Facilitate advancedplanning discussions with participants
Documentchronic conditions via HCC capture
Schedule andShift Details :
40 hours per week no oncall.Schedule will vary.
