Master Center is seeking a talented Care Navigator to work in close collaboration with the Care Team, including Licensed Mental Health Providers, Addiction Medicine Providers, and Peer Recovery Specialists.
The Care Navigator is responsible for providing care navigation services as part of the Regional Hub. The Care Navigator is committed to removing patient’s barriers to care, will direct inquiries to the appropriate resources and foster and maintain relationships in the community, as well as providing follow-up calls to referred families and providing feedback as requested to PCPs.
Responsibilities include maintaining accurate community resources, identifying barriers to treatment, assisting uninsured patients with Medicaid enrollment, onboarding new patients, and ensuring timely coordination of care.
Master Center for Addiction Medicine is a private, outpatient addiction medicine treatment center headquartered in Glen Allen, Virginia.
We have additional offices in Hampton, Gloucester, Chesterfield and are opening offices in other areas of Virginia. Our approach to addiction is comprehensive, evidence-based and individualized, drawing on every tool that has been proven effective : medical treatment, education, psychiatry, professional counseling, testing / monitoring and peer support services.
Our aim is not just to treat patients but to heal them by helping each one find his or her way to lifelong recovery. DUTIES AND RESPONSIBILITES : Develop and maintain relationships with intake coordinators at community substance use disorder agencies, clinics, and organizations.
Participate regularly in community-based meetings to stay current on new and evolving resources in their APAL region. Research and maintain accurate and up to date lists of local substance use disorder resources and their availability, including substance use disorder providers, family support services, community resources, peer support programs, related programs in the APAL database.
Each resource should be verified at least annually; availability of resources may vary by region. Answer APAL PCP calls and emails during normal business hours and direct to appropriate resources Provide follow-up calls to referred families with designated timeframes in order to ensure continuity of care.
Provide feedback as requested to PCPs on status of family engagement and services. Record and log information from daily calls into APAL databases.
Conduct post-navigation surveys to gather data from PCPs and families regarding satisfaction of process. Adhere to HIPPA requirements, as applicable.
Accept supervision by the APAL Regional Licensed Mental Health Provider and Addiction Medicine Provider. SKILLS : Strong organizational abilities, including effective time management skills, ability to manage multiple tasks at the same time and respond to requests in a timely manner.
Writing skills and ability to thoroughly document work on an ongoing basis. Strong communication and interpersonal skills in order to effectively engage with a diverse range of providers, agencies, and families.
Strong customer service skills, particularly while engaging over the phone. Be proficient in Word, Excel, data entry, and editing.
Demonstrate flexibility and willingness to learn. Preferably, have lived experience navigating healthcare systems and / or mental health services.
QUALIFICATIONS : Minimum : A bachelor's degree from an accredited institution. Preferred : Bachelor’s degree in human services field.
Minimum : 2 years of experience working in the community substance use disorder system in Virginia. OR Minimum : A high school diploma.
