Position Title : RN - Care Center PRN
Overview : RN provides a full range of hospice nursing care to patients and families in care center, facilities and home settings.
This position monitors patient's medical conditions and provides accurate reports to attending physician. They also coordinate case management of patient services and medical care, work in an interdisciplinary care setting to teach, assist, and support patient / family with the dying process.

Qualifications :
- Florida RN license, required.
- Requires a current Cardio Pulmonary Resuscitation (CPR) certification (American Heart Association).
- Oncology, palliative care, geriatrics, or home health care experience preferred.
- Experience working with interdisciplinary team desired.
- Current Florida driver's license and insurance.
Location : Orange Park Care Center
The Orange Park Care Center is Haven’s fifth free-standing care center serving north Florida is a state-of-the-art facility that allows us to expand our services to better meet the end-of-life and palliative care needs of patients and families in the area.
The care center is located at 745 Blanding Blvd, Orange Park.
