Position : Foster Care Case Manager
Location : Northeast Ohio (Hybrid)
Type : Full-Time

Key Responsibilities :

- Foster Family Support : Serve as a primary resource for foster families, providing them with guidance, resources, and emotional support.
- Assessments & Planning : Conduct home visits, assessments, and service planning to ensure each child's needs are properly addressed.
- Collaboration : Work closely with the legal system, schools, medical providers, and other agencies to advocate for the child's best interests.
- Compliance & Documentation : Ensure that all case notes, files, and reports are completed accurately and in a timely manner to meet legal and organizational requirements.
Qualifications :
- Licensed Social Worker (LSW) in the state of Ohio (Required).
- Bachelor's Degree in social work or a related field.
- Experience working with children and families, preferably in foster care or a related area.
- Strong communication, organizational, and time management skills.
- Ability to work independently and as part of a multidisciplinary team.
- Valid driver's license and access to a reliable vehicle for travel.
