Employment Type : Full-Time, Monday to Friday, No Weekends

Pay Range : $25.00 - $27.00 per hour

Expected Hours : 37.5 hours per week

Job Description :

We are seeking a dedicated and professional Licensed Practical Nurse (LPN) to join our team. The ideal candidate will have a strong background in nursing care, excellent communication skills, and the ability to work independently.

This role involves providing direct nursing care under the supervision of a Registered Nurse (RN) and following established care plans.

You will also be responsible for documenting patient progress, administering medications, and educating patients and their families.

Key Responsibilities :

• Provide health education according to the teaching plan established by the RN.
• Deliver routine nursing care as outlined in the nursing care plan and within the LPN scope of practice.
• Document services provided, changes in patient condition, and other pertinent observations.
• Measure and record vital signs and collect routine patient data.
• Identify high-risk situations and make appropriate referrals.
• Administer medications accurately and transcribe physician orders.
• Adhere to infection control policies and maintain a safe environment.
• Assist in the movement and transportation of patients when necessary.

Qualifications :

• Education : Graduate of an accredited school of Practical Nursing.
• Experience : Minimum of 1 year as an LPN.
• Licensure : Current Practical Nurse license in the State of New York.
• Skills :
• Strong computer skills and proficiency with EMR systems.
• Excellent interpersonal, organizational, time management, and communication skills.
• Ability to complete paperwork accurately and in a timely manner.
• Knowledge in measuring and administering medication.
• Requirements :
• Must comply with HIPAA and privacy regulations.
• Ability to lift a minimum of 50 lbs and stand or walk for long periods.
• Must possess a valid NYS driver's license and a reliable vehicle with insurance.
• Willingness to travel throughout the assigned county.
• Must be willing to relocate to Rensselaer, NY before starting.
