PURPOSE OF THE POSITION
The purpose of this position is to ensure satisfaction of clients through delivery of quality care and care coordination with more immediate starts of care and less disruption with callouts.
SCOPE
The Team Lead reports to the Agency Director and is responsible for ensuring the satisfaction of both the clients and the caregivers.
The incumbent’s responsibilities include a broad range of interaction with caregivers and clients.
QUALIFICATIONS
Must be 18 years of age or older
Must be able to speak, read and write English
Must not have a criminal record which would bar employment
Must be able to tolerate a physical workload including standing or sitting for prolonged periods of time
Must be able to tolerate a variety of housing conditions
Must be able to work at least every other weekend
Must have a valid DL and auto insurance
Must have transportation in good working order
Must have demeanor and social skills that would enable them to communicate well with clients to achieve favorable outcomes
At least 2 years of experience in caring for individuals with functional disabilities and a current CNA certificate
RESPONSIBILITIES
Field / Office operation Perform the duties of a caregiver as assigned.
Conduct caregiver training in mentor role for employees needing additional or specific client
Assist with delivery of supplies and documentation as needed to field staff.
Conduct supervisory visits to ensure customer satisfaction; notifying Care Manager of any necessary updates in the individual service plan;
or resolve issues as necessary including completing infection control reports etc.
Participate in weekly team
Perform other related duties as

WORKING CONDITIONS
Physical Demands
The Team Lead will spend hours in the office and in clients’ homes. The position requires ability to lift 50 pounds, ability to drive a motorized vehicle and to sit / stand for prolonged periods.
Environmental Conditions
The incumbent is in a busy office or in client's home. He or she is faced with constant interruptions and must meet with others on a regular basis.
Mental Demands
The incumbent must deal with a wide variety of people and client / caregiver issues daily.
Work Hours
7 : 00a to 8 : 00p up to 40 hours, Monday through Friday with 2 days off during the week every other week and every other weekend
Occasional evenings be required
On-call duties will be regularly required
Pay Rate : $18 / hour
