The schedule for this position is daytime hours, 8 : 30 a.m. to 5 : 00 p.m.. The position is located in Clackamas at our Talbert Health Center.

The Care Coordinator collaborates in direct client coordination, with both medically complex conditions and severe mental illnesses and addiction issues.

This work is done while supporting a Trauma Informed Care environment. With the high prevalence of trauma among individuals receiving behavioral health services, it is required that the Care Coordinator understand the effects of trauma on health, coping, and other aspects of the lives of those we serve.

The Care Coordinator for the Integrated Care team serves as an integral part of a larger treatment team and assists in coordination across disciplines that as a whole provide a wide range of supportive, therapeutic mental health, and primary care services to shared clients.

This position assists in linking to and coordinating between medical and psychiatric services. The position will contribute to the work of the multi-disciplinary team, including both medical providers and behavioral health clinicians.

This position involves supporting the interdisciplinary staff in promoting a strengths-based, client centered approach to assisting individuals in achieving their goals as they pertain to both medical and behavioral health needs.

Essential Duties :

  • Work with inter-disciplinary team to promote coordination of service delivery and prevent duplication of services.
  • Coordinate referrals with other client providers in a timely and professional manner. Collaborate with inter-disciplinary team and community services to ensure that any diseases, as well as preventative and health promotion needs are addressed as part of the holistic, whole-person, culturally-sensitive physical health approach
  • Coordinate client schedules to include arranging appointments, confirmation calls, and follow-up for no shows. Includes explaining the purpose of appointments to the client in clear and simple language.
  • Assist clients during their physical medical appointments in understanding the relevant details of the visit, as requested by either the patient or inter-disciplinary team.
  • Provide weekly client appointment updates to treatment team with follow-up planning.
  • Identify and prioritize service plan goals and identify resources to meet those goals.
  • Identify and engage clients in health care services, and / or coordinate and monitor their health conditions already being addressed (ie.

asthma, diabetes, heart disease, obesity).

  • Teach and assist clients to better manage their chronic diseases using realistic and specific, measurable, attainable, realistic and timely goals.
  • Advocate and collaborate the completion of applications for clients including securing benefits and accessing appropriate housing, transportation, medical, and emergency services.
  • Provide outreach and engagement to clients in need of additional support.
  • Coordinate with community agencies for outreach / engagement. Ensure, establish and maintain good working relationships with community agencies.

Assist clients in developing and accessing natural community supports.

Qualifications

The incumbent must possess a Bachelor's degree in a behavioral science field from an accredited college or university or three years of full time professional experience working with adults with severe and persistent mental illness.

The job incumbent must demonstrate the competencies as outlined in OAR 309-016-0005 to become credentialed as a Qualified Mental Health Associate (QMHA).

  • The CADC credential is desired given the high occurrence of substance use / abuse.
  • Clinical experience providing outpatient services or working in community health, primary care is preferred.
  • Ability to demonstrate working knowledge of accepted treatment practice with the capacity to implement and evaluate the effectiveness of treatment plans and recommend alternate strategies as necessary
  • Ability to integrate best practices into the day to day operations, including Trauma Informed Care and Motivational Interviewing
  • Understanding of recovery model, motivational interviewing, illness management and recovery
  • Thorough knowledge of evidence based practices, and strength based case management philosophies and strategies
  • Ability to operate computer hardware and software at a level needed to effectively perform job functions , enter and retrieve data and prepare simple reports
