Licensed Vocational Nurse (LVN) - Home Health Care

Therefore, you will always have as many visits as you desire.

REQUIREMENTS :

• Availability of at least two days, or ten productivity units a week and one weekend a month for per diem. Visits are typically conducted between the hours of 8am-5pm.
• Reliable transportation, current driver’s license, auto insurance
• At least one year experience with direct patient care
• Able to perform basic wound care, blood draw, and catheter care (OR willingness to learn these skills)

ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES :

• Make home visits to clients in the community, as well as Senior Living, Assisted Living, and Independent Living facilities.
• Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
• Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environment for hazards or barriers to more independent living.
• Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
• Continually assess and revise the occupational therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

$45 Follow-Up Visits requiring wound-vac

Additional Field Pay*

Mileage Pay - $0.58 per mile (Milage is paid for travel following your first visit and in between subsequent visits, it does not include the trip from your home to your first patient and your final patient to home)

Data / Cell Phone Usage Pay - $0.25 per visit
