GRANT SUMMARY

The goal of the Healthy Transitions project is to improve existing and expand access to developmentally, culturally, and linguistically appropriate services and supports for transition-aged youth and young adults (ages 16-25) who either have, or are at risk for developing, serious mental health conditions.

The overarching goal is to improve emotional and behavioral health functioning so youth and young adults can maximize their potential to assume adult roles and responsibilities to lead full and productive lives.

POSITION SUMMARY :

This position is responsible for the day-to-day operations of the project, performing general management functions including project planning & development, personnel management, and assisting with or performing budgeting and procurement of funds management.

JOB DUTIES

  • Improves the life trajectory of American Indian and Alaska Native (AI / AN) transition-aged youth and young adults (TAY&YA) by improving outreach, identification, and enrollment to culturally appropriate behavioral health services and supports.
  • Improves cross-system collaboration, service capacity, and expertise to improve services and supports for TAY&YA.
  • Creates, implements, and expands project services and support.
  • Addresses the need to engage the target population in project services and support through recruitment and cross-provider training.
  • Develop, with established skilled support, an Interagency Agreement (IAA) to identify and detail measurable outcomes, develop a cross-system partnership of stakeholders, ensure collaboration in services provided, develop a Sustainability Plan, develop Policies and Procedures, develop how evidence-based practices will be selected, and develop strategies for suicide prevention.

Updates frequently with the established skilled support stakeholders.

  • Develop a written plan describing outreach, evidence-based screening and assessment, referral pathways, and recovery support services.
  • Monitors Substance Abuse and Mental Health Services Administration's (SAMSHA) reports to improve the project processes and make SAMSHA Project Officer-approved adjustments.
  • Uses evaluation data to enhance services, improve quality, and track performance targets ensuring project targets are met.
  • Ensures all reports are completed and submitted as required. Reports include qualitative and quantitative data; progress addressing quality care related to Disparity Impact Statement (DIS);

barriers encountered; efforts to overcome barriers; evaluation activities for tracking DIS efforts; and revised quality improvement plan if the DIS does not meet the quality standards.

  • Verifies screening, assessment, diagnostic, evaluation services, crisis emergency services, family services, and respite care has been provided to consumers.
  • Develop an Annual Sustainability Plan in coordination with the BHCC.
  • Oversees and makes recommendations for the training of service providers.
  • Ensures all grant requirements are met by overseeing contract deliverables with responsibility for achieving project goals and objectives.
  • Oversees daily operations of project duties, goals, and objectives in a timely manner.
  • Supervises project to ensure all services are providing culturally appropriate, trauma-informed care and are guided by evidence-based practices.
  • Monitors project budget.
  • Ensures performance improvement and quality management by participating in continuous quality improvement and data-driven decision-making processes to assess program efficacy and drive quality improvements.
  • Ensures adherence to data collection requirements.
  • Implements project services in a timely manner.
  • Develop continuation grants.
  • Develop all grant reports per the Administration for Native Americans (ANA) requirements and submit to program management before submission to the granting agency.
  • Meets with the Project Evaluator quarterly while overseeing quality assurance.
  • Attends ANA grantee meetings.
  • Performs other job-related duties as assigned.

NECESSARY KNOWLEDGE, SKILLS, AND ABILITY

  • Ability to report to work on time.
  • Above average skills in operating / using computers and using Windows, Word, and Excel with the ability to accurately input information into reporting formats.
  • Above average communication and people skills with the ability to accurately follow written and verbal instructions.
  • Knowledge of how to best assist possible escalated individuals during the data collection process.
  • Strong understanding and sensitivity to diverse cultures and lifestyles, with demonstrated experience working effectively with Alaska Native populations.
  • Ability to protect confidentiality is essential; HIPAA & 42 CFR compliant.
  • Knowledge and ability to interpret / understand billing procedures.
  • Skill in managing multiple priorities and tasks concurrently and meeting their deadlines.
  • Skill in establishing and maintaining cooperative working relationships with other employees.
  • Ability to work independently as well as with teams.

MINIMUM QUALIFICATIONS (Education & Experience)

  • Bachelor's degree in behavioral health or related field.
  • One (1) year of experience working in behavioral health.
  • Two (2) years of experience working with AN / AI population and experience providing culturally specific services.
  • Three (3) years of experience working in project management.
  • A combination of experience, education, and training that provides the required knowledge, skills, and abilities may substitute for degree requirements.

PREFERRED QUALIFICATIONS (Education & Experience)

  • Master's degree in behavioral health or related field.
  • Experience working with transition-aged youth and young adults.
