The VNA of Eastern Massachusetts is currently seeking a full time Physical Therapist and Physical Therapy Assistant . Our home care program is growing and partnering with other community providers to bring comprehensive healthcare directly into the home, from newborns to centenarians, we are striving to improve the health of the entire community.
We are looking for healthcare professionals who share this devotion for caring for others.
Physical Therapist
Job Summary
- Completes admissions, evaluates, provides treatment, initiates referrals, plans and completes discharges for assigned patients.
- Provides introduction, teaching and supervision of Home Health Aides as appropriate.
- Includes patient and family / caretakers in goal setting, treatment process and discharge planning.
- Schedules patient visits efficiently in relation to patient needs and meets productivity expectations.
- Initiates, maintains and documents effective communication with other care providers, district office personnel, and supervisory staff.
- Complies with agency policies and district procedures for clinical and administrative documentation.
- Participates in formal and informal case conferences and consultation.
- Demonstrate ongoing communication with Physical Therapy Assistant, physicians, other disciplines, and community agencies via narrative or written case conferences, documentation and team meetings.
- Follows district and agency procedures for obtaining and maintaining durable medical equipment and therapy supplies.
- Performs other duties as assigned and complies with all policies and procedures of the Agency and other regulators.
- Attends departmental and committee meetings.
- Participates in activities that enhance self-competence and the agency’s performance improvement program.
Qualifications and Experience
Qualified candidates must have :
- Current physical therapy licensure and CPR certification
- Minimum 1 year experience preferred in acute care, rehab or homecare
- Excellent clinical skills
- Awareness of problem-solving techniques
- Working knowledge of rehab diagnosis and therapy process
- Verbal and written communication skills
