What Will You Do?
The Clinical Care Team Coordinator administratively supports the RN and MSW Navigators in the field as well as the participants in the program.
Responsibilities
- Provide phone support for participants and other outside agencies
- Administrative assistance related to client coordination of care
- Support Intake Department with referrals and enrollment needs
- Gather required health care information from PCPs and other agencies
- Review billing for financial and clinical compliance
- Maintain current database of agency clients and employees and insurance authorizations

Education and Experience
- Medical Assistant or other clinician such as EMT, LPN, etc for at least 2 years
- Strong skills with MS Office software as well as office equipment and machines
- Customer service experience in a health care setting highly valued
- Proof of a valid Michigan driver’s license, auto insurance and reliable transportation
