The Licensed Practical Nurse reports to the Team Manager of North Kansas City Hospice House.
He / she is responsible for providing nursing and supportive care for the terminally ill patient and their families in accordance with agency policies.
Functions : Responsible for implementing the care plan for patient / family unit.
Updates interdisciplinary team at patient care conference regarding patient / family needs.
Provides direct nursing care to patients in order to maintain highest level of care for the patient under direction of the primary physician and registered nurse.
Uses problem oriented charting to ensure continuity of care in a home setting as well as inpatient setting.
Teaches family members patient care skills, including : pain control, medication administration, dressings, hygiene, nutrition, checking vital signs in accordance with orders from physician, as well as addressing emotional needs.
Assists patients with activities of daily living and personal needs : bathing, range of motion exercises, elimination needs, eating and medications that are patient administered.
Maintains a clean and neat patient environment and does light housekeeping as indicated in the plan of care.
Practices safety precautions with durable medical equipment in the home.
Attends conference and inservices as requested by the supervisor Ensure that arrangements for equipment necessary in the home setting is made available.
Job Description Will attend to patient and family at the time of patient's death.
Is knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection.
Teaches patient / family infection control protocols.
Uses resources efficiently and prudently, i.e.
medical and office supplies, etc.
Schedules or manages his / her time effectively Communicates with patients & families, referral sources, insurance case managers, physicians and others with courtesy, responsiveness and cooperation.
