Founded in 2009 by the Housing Authority of the County of San Bernardino, Knowledge and Education for Your Success (KEYS) is a 501(c)(3) nonprofit organization, private-public partnership, dedicated to enhancing access to supportive services for low-income residents in San Bernardino County.

KEYS provides an array of services including case management to assist families and individuals in accessing existing resources and services within the County.

In summary, the SERVICES COORDINATOR will provide service coordination for all KEYS PROGRAMS. The SC will work in partnership and collaboration with multiple entities to assist in rapidly rehousing and / or achieving stabilization for homeless individuals and / or their families.

Current available position would be working out of the high desert but available to come to San Bernardino offices as needed.

Key Responsibilities include :

  • Verify eligibility of referred homeless individual and / or family to participate in KEYS' program(s).
  • Assess families for strengths and barriers, including employment, mental health, education, etc., and coordinate the appropriate services.
  • Develop and monitor Individualized Service Plan (ISP) based on the assessment, by completing appropriate forms with customers to identify specific actions and / or resources needed for successful stabilization of the customers.
  • Coordinate and refer services with other social service agencies to provide mental health, medical, dental care, and / or rehabilitation services.

Link customer to services in the community for mental health treatment, and physical health as needed. Leverage resources available within the community including faith-based organizations as appropriate to the situation in order to benefit the customer.

Meet with customers on a regular basis in office or in the community.

EXPERIENCE / TRAINING / REQUIREMENTS

Bachelor's degree in social work, human services, counseling, other behavioral science, or similar fields preferred. Equivalent experience with education will be considered.

Must have at least two years' experience working in a community with disadvantaged individuals or those at high-risk for homelessness.

Previous experience with customers with substance / drug abuse or mental health issues a strong plus. Bilingual in English / Spanish preferred.

Position does require auto insurance levels for bodily injury at $100,000 / $300,000. Set Auto Stipend will be provided monthly to offset costs for insurance and mileage.
