The Behavioral Health Community Support Team (CST) Case Manager (QMHP) is responsible engaging and assisting clients with mental health needs in a community based setting.
Services are provided in the community in a variety of settings such as client homes, homeless shelters, local hospitals, other agencies, medical offices, and shopping centers.
The CST case manager works on a team of case managers / service providers within the agency and throughout the community to ensure clients’ needs are being met and safety is maintained.
What you may do in a day :
- Provide community based support to clients with mental health challenges and support clients with effectively managing their mental health needs.
- Assist with independent activities of daily living such going grocery shopping, attending medical / psychiatric appointments, medication training, social skills training, money management, and coping skills, etc.
- Provide crisis intervention services as needed to de-escalate clients within the community or via phone.
- Complete initial and semi-annual mental health assessments to assess current needs and strengths in order to develop an individual treatment plan for clients.
- Complete necessary documentation for services provided following Rule 132.
- Collaborate with staff to coordinate services and ensure client’s needs are met.
What will you bring to the table :
- Master's Degree in a human services field.
- At least one year experience working with mental illness
- Knowledge of the DSM V & Rule 132

- Ability to navigate stairs
- Ability to provide services in clients' homes and in the community
- Flexibility to work evening / overnight / weekend / rotating holidays
- Ability to work rotating on-call shifts
- Cover non-scheduled shifts as needed
- Use of personal automobile
- Must be able to drive a passenger vehicle
- Maintain automobile liability insurance as defined by AID’s
- personnel policies
- Maintain valid Illinois driver’s license
- Drive self and clients in agency or personal vehicle.
- May be required to work multiple locations
- Complete Training as required within 30 days of hire.
- Must acquire and maintain certifications in First Aid, CPR, Non-violent Crisis Intervention Training, CEU’s and other relevant trainings.
Shift coverage may vary and may include evening and weekend shifts.
