Job Description : FT / PT, LPN or RN, Weekday / Weekend Shifts Available
Hospice On Call Registered Nurses facilitate the prioritization of patients and families that require compassionate hands-on nursing care performed in a variety of settings such as a nursing facility or home.
Hospice nurses play a vital role in helping patients spend their final days in a way that truly reflects who each individual is and what matters to them most.
More than providing medical care, it’s about going above and beyond to honor their wishes and ensure they have dignity and comfort.

WHAT YOU WILL DO :
- Assess physical, psychosocial and spiritual needs of the patient and family (RN only).
- Gather data related to patient and family’s physical, psychosocial and spiritual needs (LPN).
- Provide intermittent skilled nursing services to the patient under the patient’s physician’s direction.
- Triage calls received to determine best course of action and addressing any outstanding needs such as DME / medication orders.
- Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
- Serve as a liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge relating to the patient’s needs.
- Provide emotional support to the patient and family.
- Refer patients and / or family members to other services when appropriate.
- Provide instruction (health care teaching) to the patient and family as related to care in a residence.
- Make visit to patient’s residence at the time of death.
- Maintain, promptly complete, and return all documentation as required.
- Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
- Relay pertinent information to appropriate team members.
- Participate in meetings as initiated by team leader.
- Participates in mandatory or elective events, trainings, or in-services as necessary.
- Perform other duties as assigned.
On Call Triage RN Qualifications & Requirement :
- 1+ years of prior HOSPICE Registered / Licensed Practical Nursing experience is required
- An active unencumbered state RN / LPN license
- Current driver's license and auto liability coverage
- CPR certification, a copy of TB Results, a copy of an updated physical
- Schedule and handle incoming calls for the night / weekend shift; past experience is required.
- Ability to provide direct patient care in homes and facilities.
On Call Triage RN Schedule & Benefits :
- Shift : Various shifts- Monday to Friday (5 pm to 8 : 30 am) OR Weekend Option- Friday to Monday (5 pm to 8 : 30 am)
