Home Health Licensed Practical Nurse (LPN)

Responsibilities :
- Provide in house nursing care for patients with various health conditions.
- Perform assessments, take vitals, administer medication, and document patient progress.
- Educate and support patients and their families in managing illnesses or injuries.
- Work closely with physicians, RNs, and other team members to execute comprehensive care plans.
Qualifications :
- Active LPN license in Tennessee and graduation from an accredited nursing program.
- Prior experience in home healthcare preferred but not required.
- Strong clinical skills, communication, and compassion.
- CPR certification and a deep understanding of home health regulatory requirements.
