About The Position
Job Title : Physical Therapist (PT) - Home Health Job Specialty : PT Home Health Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 16 : 30) Guaranteed Hours : Per contract, facility may adjust shifts as needed Experience : 1 year of Skilled Home Health Experience required License : Pending license accepted Certifications : BCLS Must-Have : Primary series COVID vaccination and one booster Job Description : Provide skilled physical therapy services in a home health setting.
- Expected to conduct up to 6 patient visits per day with a standard productivity requirement.- Maintain a 1 : 1 patient ratio during visits.
- Use EPIC Remote Client for Home Health and Rover for charting.- Ensure proper grouping of clients to minimize travel time, with a maximum daily radius of roughly 60 miles.
- Weekend work is not required; no on-call duty.- Own transportation is necessary for travel between client locations.- Flexible with floating between facilities within the state, as needed.
- Opportunity is open to first-timers; local candidates accepted within a specified radius.- Completion of required orientation and training modules prior to start.
- Dress code is business casual; scrubs are optional and no specific color is required.
