Job Details
- No candidates with an AZ home address on their drivers license will be considered - please include a copy in the submission.
- This form needs to be 100% accurate and have all request noted in the highlights section.
- Start Date should be on or very quickly after the Order Start Date listed.
- All facilities the traveler is open to working in should be listed on the submission.
- If interested in 'any' location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
- All shifts the traveler is open to working on should be listed on the submission.
- All departments the traveler is open to working must be checked on the cover page.
- All candidates submitted should be preclosed and ready to accept.
Job Requirements
- Minimum 24 months of current Level I Trauma Labor and Delivery experience - REQUIRED.
- Must have Labor / Delivery and Post Partum experience - REQUIRED.
- Certifications must be American Heart Association OR American Red Cross and active upon submission - MUST BE INCLUDED IN SUBMISSION - REQUIRED.
- No Nursys Hits - REQUIRED.
- Travel experience - REQUIRED.
Schedule Information
- RTO must be less than 7 days.
- RTO will not be approved in the first 2 weeks of assignment.
Unit Specific Information
- Resume with all facilities, facility size, beds, trauma level, and units listed.
- Skills Checklist specific to the discipline (must be less than a year old).
- Active License (verification must be less than 30 days old).
- Certifications (must be active at time of submission).
- Driver's License.
Additional Details
