Physical Therapist

Physical Therapist (PT)

Salary : $95-$105k / yr + SIGN-ON BONUS

Schedule : Full-time M-F Flexible hours

Part-time and PRN opportunities are also available)

Location : Dayton, Ohio

Requirements of the PT role :

Licensed Physical Therapist

CPR / BLS Certified

Must be willing to travel

Valid Drivers License and Auto insurance Coverage

Reliable transportation

Home health experience preferred

About the Physical Therapist ro le :

Perform treatment visits, 30-day reassessment visits, recertifications, and discharges.

Reports to the Director of Therapy

Travel to a facility, or residential location to conduct patient care.

Identify a target list, short-term goals, long-term goals, and treatment plan.

Complete subsequent follow-up visits.

Provide supervision to PTAs

Maintain a weekly caseload of 27-32 visits (full-time)

Utilize a universal electronic documentation system.

Provides, recommends, and fabricates adaptive devices or other equipment.

Educates client, family, and appropriate staff in the use or application of such equipment.

Maintains positive relationships and rapport with coworkers, supervisors, clients, family members, community personnel, and administration.
