CalWORKs Phoenix Program is funded through Alameda County's Mental Health Services Act (Proposition 63) funding and is designed to actively outreach, engage, and provide a full range of services to 30 adults in Alameda County who have mental health and substance abuse challenges and are enrolled in the Welfare to Work Program.
Position Summary :
Under the general direction of the Program Director, the Clinical Case Manager is responsible for overseeing the clinical needs of the CalWORKs! Phoenix program members.
The collaborative team provides services with an integrated framework, and the Clinician Case Manager plays a critical role in supporting a client's progress as they move through their engagement activities.
Within the CalWORKS Phoenix Program therapy is brief, time- limited to 12-14 sessions, and focused on reducing symptoms to enable client successfully pursue their goals toward employment and / or vocational training.
This position will work alongside the employment specialist and vocational case manager in efforts to support the clients' journey toward greater independence, well-being, and self-sufficiency.
Services often provided in office; however, flexibility needs to be demonstrated to meet client in the community; such as library, job, on-site, and home visits shall be scheduled when appropriate and convenient for clients.
The Clinical Case Manager is responsible for maintaining consistent contact with clients, deepening the treatment relationship, assisting clients to identify life goals and linking them to community resources that can support their wellness and recovery.
The Clinical Case Manager needs to have the skills, knowledge and ability to shape an effective working alliance, foster hope, build motivation, assess pressing needs, identify barriers to treatment, and link clients to resources to meet their basic needs.
The Clinical Case Manager must demonstrate the knowledge and clinical expertise to teach clients the skills that will support symptom reduction and enable them successfully pursue their goals, and to provide clients with regular coaching to apply these skills in their daily lives.
Successful candidates will be knowledgeable and experienced employing intervention strategies using motivational interviewing, trauma-informed care, harm reduction and abstinence-based models of treatment.
Clinical supervision is available for this position.
This position is 40 hours a week, required in-person office schedule with in-field work.
Qualifications :
- Active registration with BBS, Registered MFT or ASW.
- Must demonstrate an ability to work both independently and as a member of our cohesive team.
- The ability to prioritize effectively among competing demands on time.
- Solid written skills to meet required standards of documentation.
- Interpersonal skills including emotional intelligence.
- Must be able to meet Immigration Reform Act of 1986 requirements.
- Must have a vehicle to use for work duties, a valid California Motor Vehicle Operator's License, and automobile insurance.
Preferred :
- Bilingual (Spanish) preferred.
- Previous experience working in an Electronic Health Record (EHR)
Essential Functions / Skills :
- As a member of the clinical case management team, clinician will coordinate with flexibility and provide supportive services for 20-25 adults referred from CalWORKs Welfare-to-Work agency who reports or exhibits mental health challenges and / or barriers to employment or vocation.
- Facilitated weekly / monthly group activities that promote self-sufficiency, recovery wellness, healthy boundaries, Seeking Safety, Parenting, Spirituality / Healing Circle, Empowerment, literacy, career development, etc.
- Individually, provides psychoeducation to clients and when appropriate, family members, related to health, well-being, and lifestyle, including symptom management, medication, nutritional, sleep hygiene, trauma-informed care, and relapse prevention;
assists clients with insightful benefits of employment activities as a wellness intervention.
- Completed initial bio-psychosocial assessments, annual assessments and establishment collaborative treatment goals.
- Provide strength-based, trauma-informed, culturally humble, and non-judgmental high-quality services to meet clients' comprehensive needs, including linking clients to housing, employment, medical, dental, psychological, parenting, legal, transportation, and money management resources.
- Must have familiarity or prior experience serving underserved / high risk families.
- Ability to effectively manage and resolve crisis.
- Knowledge of and / or ability to provide assistance with community and concrete resources.
- Must be computer competent and demonstrated ability to learn EHR systems.
- Completed all clinical documentation within required timeframes.
- Attend all clinical supervision, case conferences and team meetings as scheduled.
Physical Requirements
Physical demands include continual sitting, typing, and repetitive computer use for up to 8 hours a day. The position also requires frequent communication with others, including hearing and speaking clearly with individuals and groups, in person and via phone and video calls.
Ability to lift 50 pounds at times. Regular local travel and operation of a motor vehicle is required. These requirements are representative of those that must be met to successfully perform the job's essential functions.
