The Position :
St. Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager for Project LIVE, to provide client support in our Permanent Supportive Housing Program.
The Case Manager will offer housing-based case management support and advocacy to program participants who are chronically homeless, among other duties.
As a part of the Project LIVE team, this case manager is responsible for creating treatment plans, collaborating with program managers, and providing stability to clients.
Moreover, the Case Manager will need to have experience working with the unhoused population and conducting home visits.
The core working hours are Monday through Friday from 8 am to 4 : 30 pm, and the wage is between $20.00-$25.00 an hour. Please see full job description for additional details https : / / www.
